Provides community housing and supported living services for all people with disabilities and/or serious mental health diagnoses, assisting them to maintain an independent life. Services include working on daily independent living skills, interpersonal and communication skills, coping and social skills and skills in the use of community resources. Services are offered in a variety of settings, which include group homes, supervised apartments, and supported apartments.

Group Homes - provide opportunities for individuals with disabilities to increase their independence through 24/7 guidance and supervision. Staff assists residents in developing essential daily living skills, such as cooking, shopping, housekeeping, and money management, medication training and self-management of their conditions, and interpersonal and behavior management skills. Each resident has their own bedroom and share common areas.

Supervised Apartment Living - provides a high level of supervision with staff being on site 24 hours/day and are delivered to individuals according to their specific needs. Staff provide daily living and life skills training, medication monitoring and training, case coordination, supportive counseling, skills training and education, behavior management skills, and community integration activities.

Supported Living Services - provide intermittent onsite support during the day and evening hours and may be provided in Bridgeway owned apartments or in the consumer’s own home. Staff provide daily living and life skills training, medication monitoring and training, and community integration activities.

Provides a variety of residential programs for those who have a developmental, vocational, emotional, mental, or physical disability. Programs include the following types of residential services:

- Community-Integrated Living Arrangement (CILA): a living arrangement provided to Adult Waiver participants in a group home, family home or apartment where eight or fewer unrelated adults with developmental disabilities reside under supervision of the community developmental services agency. Residents receive a comprehensive individualized array of residential options, personal support services and supports under the direction of a community support team within the local agency. Funding comes through DHS Division of Developmental Services.

- 24 Hour CILA: these homes have staff on-site at all times Individuals are present. Staff provide both scheduled and unscheduled supports and services as needed by Individuals.

- Intermittent CILA: serves Individuals whose service plan documents that they do not require 24-hour staff presence, and documents the situations in which individuals may safely be away from staff direct supervision. Intermittent Residential services in apartments or family homes have staff available on call 24 hours per day. On-site staff are available to provide both scheduled and unscheduled supports and services as needed by the Individuals served and as specified in each Individual’s service plan.

- At Home Day Program: part of the residential program for Adult Waiver participants. At Home Day Programs are based in the individual’s CILA home and provide a structured personalized program of in-home and community activities for Individuals who are unable to participate in out-of-home day programs.

- Group Home: a living arrangement provided to participants in a group home, family home or apartment where eight or fewer unrelated adults with mental illness and /or developmental disabilities reside under supervision of the community services agency. Residents receive a comprehensive individualized array of residential habilitation, personal support services and supports under the direction of a community support team within the local agency. Funding usually comes through DHS Division of Mental Health.

- Apartments: a living arrangement where the person has their own relationship with a landlord and is basically living independently. Some supervision can be provided if the person wishes the assistance. Case management and case coordination services are always available on a as needed basis.

- Own Home: a living arrangement where the Individual owns their own home and is living independently on their own or with family. Some supervision can be provided if the person wishes the assistance. Case management and case coordination services are always available on an as-needed basis.
