The Care for Kids Program - an Early Periodic Screening, Diagnosis & Treatment (EPSDT) program - serves residents of Polk and Dallas Counties. EPSDT is the benefit package of Medicaid/Title 19 for individuals under 21 years of age.

The focus of the EPSDT Care for Kids program is to inform families of the benefits they receive with Medicaid/Title 19. -help individuals secure a medical and dental home. Ensure that eligible individuals receive and understand the importance of preventative health care. Link families to other community resources to improve their health and well-being (WIC, Early ACCESS, parenting classes, child care assistance, housing assistance, food pantries, etc.).

Provides transportation to and from non-emergency medical appointments, grocery stores, food banks and farmers markets.
Provides a limited number of rides to senior citizens. Must call one day in advance; Limited to 6 round trips or 12 one way trips per fiscal year July 1st - June 30, Transportation is on a case by case basis and limited to trips for medical appointments and pharmacy pick ups/drop offs.

Provides transportation to any person 60 years or older with pick-up site in Logan or Mason County. Transportation Services can be obtained for regular trips to medical appointments or treatment. Trips must be scheduled 24 hours in advance.

Please note: Persons under the age of 60 can utilize transportation but must pay the full fare (suggested donation). Persons over the age of 60 take priority.

Advises and aids veterans and their dependents in applying for federal benefits through compensation or pension claims. Emergency financial assistance for basic necessities such as rent, mortgage interests payments, utilities, and burial expenses may be available. Can also assists with applications for benefits made available by federal, state or local laws.

Provides public transit services to all citizens and visitors of Boone, Dallas, Jasper, Madison, Marion, Story, and Warren counties. The service is open for anyone for any reason regardless of age, income, or disability status. All HIRTA vehicles are ADA-accessible.

Rides free of charge for cancer patients to and from cancer-related appointments. Two day notice is recommended. Rides are based upon volunteer availability.
Volunteers provide companionship, access to food resources, and medical transportation to eligible community members. Medical transportation is available to other counties as well, including transportation Des Moines and Iowa City. Volunteers can serve at HIRTA Helps' volunteer stations, which are local public agencies and nonprofits. Volunteers receive mileage reimbursement and supplemental and liability insurance.

Offers preventive healthcare services for eligible children ages 0 through 20, including oral healthcare.

Helps Iowa families with children and teenagers (birth through 20 years) find medical, dental, vision, and mental health providers; schedule health care appointments; connect to community resources; screen to see how young children are learning and growing; and enroll children in Hawk-I and Medicaid health insurances.

Provides volunteer drivers in to Story County residents with transportation to and from medical appointments and other essential services. Although not exclusive, our priority is serving Story County's older adult population.
