Offers women’s health services to support women through every stage of life. Services include annual wellness exams, breast exams, pap smears, pregnancy care, reproductive health and birth control, menopause care, on-site ultrasounds, and routine tests and screenings.

Provides in home support services for individuals in need. Services include personal care, light housekeeping, meal preparation, exercises and a respite care program. 

Provides outpatient mental health therapy, medication management, and psychiatric assessments. Services are offered to adults with disabilities, including individuals with Mental Illness, Intellectual Disability, or other Developmental Disabilities, who require support in multiple areas. 

The program integrates Medicaid funding and county regional funding to assist individuals in transitioning back into the community after hospitalization for acute or chronic mental or emotional disturbances. 

Eligibility requires qualification for Polk County Regional funding; individuals may or may not be eligible for Medicaid.

Provides general medical care for veterans and their families. Services include checkups, prevention, diagnosis, treatment of diseases, immunizations and vaccinations, labs and blood work.

Offers mental health services. Services include, general counseling, teen services, alcohol & addiction services, marriage counseling, domestic violence/sexual assault services, psychiatry  and telehealth/virtual services.

Provides health care and specialty health care services, including mental health care. Can also provide family and internal medicine services.

Offers to help patients and their loved ones to prepare a personal, comprehensive plan of care that meets needs and helps to reach end of life goals. Offers bereavement, grief/grieving support groups.

Provides children and adult immunizations.

Works to prevent disease, promote health, and prolong life among the population as a whole. Services include immunizations, communicable disease information, and environmental health hazards information.
