Under the direction of a physician, a team of health care professionals will develop a plan of care based on individual needs. Services offered to home health patients include but are not limited to:

- Wound and Ostomy Care.
- Diabetes Management.
Registered nurses are available 24 hours a day by phone to provide specialty care for patients with virtually any medical diagnosis.
OSF Home Care also offers:
- Diabetes Supply Program which provides comprehensive, convenient supply services and home delivery of diabetes products.
- Home Infusion Pharmacy which specializes in providing home intravenous therapies, clinical therapy management, and other related support services not available from most traditional retail pharmacies.
