Under the direction of a physician, a team of health care professionals will develop a plan of care based on individual needs. Services offered to home health patients include but are not limited to:

- Case Management.

- Telemonitoring.

- Rehabilitation Services.

- Supportive Counseling.

- Pediatric Care.

- Intravenous Therapy.

- Pain Management.

- Wound and Ostomy Care.

- Diabetes Management.

Registered nurses are available 24 hours a day by phone to provide specialty care for patients with virtually any medical diagnosis.

OSF Home Care also offers:

- Diabetes Supply Program which provides comprehensive, convenient supply services and home delivery of diabetes products.

- Home Infusion Pharmacy which specializes in providing home intravenous therapies, clinical therapy management, and other related support services not available from most traditional retail pharmacies.

Provides health-related services, such as administering oral medications, help with simple prescribed exercises, keeping patient’s rooms neat, and help patients to move from bed, bathe, dress and groom

Integrated Health Home consisting of a care team of three including Nurse Care, Care Coordinator and Peer Support Specialist. Services provided to adults and children.

Provides general medical care clinics to anyone in need.

Provides public health nursing services, including adult immunizations and communicable disease follow-up.

Accepts walk-in ill visits for persons experiencing symptoms of the following minor, acute illnesses:

-- Sinus Infection

-- Pink Eye

-- Earache or Ear Infection

-- Seasonal Allergies

-- UTIs

-- Swimmer's Ear

-- Diarrhea

-- Vomiting

-- Nausea

-- Sore Throat

-- Tension Headaches

-- And many more.

Although walk-in ill visits are accepted, scheduled patients will take priority.

Provides the following services: home health care (skilled nursing, home care aids, homemakers), immunizations, dental services, lead poisoning prevention, lead testing, physical therapy, speech therapy, and occupational therapy. Additionally, the program provides communicable disease information and control.

Provides a full service hospital including a Level II Trauma Center, heart and vascular services, surgery suites for both in patient and outpatient procedures, epilepsy monitoring unit, intensive care units, pediatric services, and rehabilitation services, labor and delivery services.

Free primary care visits for the uninsured, health care services including physical exams for men, women, and children, health education, flu shots, and mental health screenings.

Preventive care and screening for diabetes, high blood pressure, daycare worker and camp physicals. Chronic disease management. 

Vaccines available for adult individuals that have no insurance or under insured. The agency asks for a $10 donation at time of service.

Offers supportive care. Patients may receive visits or phone calls from a nurse practitioner, a nurse, or a social worker. Supportive care offers visits at FHN offices and at FHN Memorial Hospital, in long-term care facilities, and in the home for those unable to travel to the clinic due to physical disabilities or distressing symptoms.

Offers home health services include nursing care and various therapy services.

Provides hospital services, including primary care, specialty care, virtual care, and more.

Provides a variety of services both inpatient and outpatient, and hospital services.

Provides critical access, general Inpatient hospital services, outpatient Services, diagnostic Imaging, laboratory, general surgery, and has a 24 hour Emergency Room.

Provides childhood immunizations, including Polio, Diphtheria, Tetanus, chicken pox, seasonal flu, Covid- 19 (for age 5 and above), and more.

Adult immunizations, including Pneumonia, Meningitis, Hepatitis A and B, Shingles, seasonal flu, Covid-19, and more.
