- Peer mentoring, where people with disabilities support each other in their daily lives.

- Housing counseling assists people with disabilities in locating, acquiring, and maintaining safe, affordable, accessible housing.

Provides clinical services that include blood pressure screenings, head lice screenings, lead screenings, TB testing, and travel consultations. 

Provides health care services in the home by registered nurses. Services include infusion therapy, central venous catheter care and injections, wound and catheter care, gait training, and health education. Also offers physical, occupational, and speech therapists, medical social workers, certified nursing assistants, and telehealth nursing.

Pediatric Home Nursing: Provides care according to the patients’ needs and the doctor’s orders, including both extended stay and skilled visits. Provides in-home care for complex pediatric clients such as those using ventilators, IV and TPN needs, trachs, g-tubes, central lines, respiratory medication, and monitoring equipment.

Health-Related Clinics: Offered in various locations, including seasonal influenza and pneumonia vaccinations, foot-care clinics, and community wellness screenings.

Hospice Care: Provides medical care, symptom and pain management, and support to persons with a limited life expectancy and their families. Registered nurses, chaplains, home health aides, volunteers, social workers, and homemakers provide service in the home or in a residential hospice facility setting.

Palliative Care: Provides pain and non-pain symptom management for patients with serious and life-threatening illness. Consultative services are provided with care centered on the needs of the patient and family, providing counseling about the disease and assistance in identifying the patient's personal goals for care.

Congregate and home delivered meals are served in Polk and Shelby on weekdays. Home delivered meals are distribed in Osceola on weekdays and Stromsburg every day of the week.

Centers have holiday and special meals as well as potluck meals once per month. Exercise and nutrition programs are available as well as free blood pressure checks by Polk County Health Department. Pool and pitch tournaments take place monthly.

Provides sexually transmitted disease testing, treatment, and education, including HIV testing to identify individuals infected with the Human Immunodeficiency Virus (HIV) and assess disease progression.

Services may include pre-testing and post-test counseling, offering information about HIV/AIDS, risk reduction strategies, emotional support, and referrals to related services.

Focuses on the prevention of HIV and STI transmission through education and outreach efforts that include topics on lifestyle and healthy outcomes.

Services include illness/minor injury care, well child exams, school/sport physicals, employment physicals, lead testing, patient assistance program enrollment, transportation, chronic disease management, diabetes/blood glucose testing, and high blood pressure testing.

Provides free and confidential services to pregnant women, their families, and individuals with young children. Services include pregnancy testing, pregnancy counseling, limited ultrasound, ongoing pregnancy support, referrals for community support services, support after an abortion, the Earn While You Learn Program, fatherhood mentoring, and a maternity home.

Also, provides childbirth education classes to prepare prospective mothers, birth partners, and family members for the labor and birth process, covering topics such as anatomy, breathing and relaxation techniques, delivery options, and postpartum care.

Additionally, resources and guidance are available to support economic self-sufficiency, including vocational evaluation, training in marketable skills, and referrals for employment opportunities tailored to those facing emotional challenges, substance abuse issues, or disabilities.

Offers programs focusing on disease and injury prevention for older adults and caregivers.

Clinic services provide comprehensive foot care, health assessments, health screenings (which include blood pressure, cholesterol/triglycerides, glucose, bone density), ear wax removal, medication, and safety checks.

Offers fitness and health education classes and physical activity programs for community, church, and social groups. Programs cover illness and injury prevention, physical activity, nutrition, and mental wellness.

Offers free, limited medical services, including:

- Medical-grade pregnancy tests,

- Ultrasounds,

- STI screening and treatment for Chlamydia and Gonorrhea for female clients and their partners,

- Pregnancy confirmation services,

- Prenatal education consultations,

- Birth control education consultations.

Also offers referrals to the Life Services Department (3171 North 93rd Street), which provides access to free material aid such as diapers, wipes, formula, car seats, portable cribs, and other child care materials, parenting education, professional counseling, and social work assistance when needed.

Provides health care services to those experiencing acute or chronic pain and/or illness, or those seeking medical services that are not due to an emergency. Services include annual screenings or check ups, chronic disease management, immunizations, pregnancy tests, pre and postnatal care, school and sports physicals, sexually transmitted infection tests, and well-child exams.
