- Skilled nursing works with the physician to update the patient's plan of care, evaluates the patient's needs, performs skilled nursing procedures, educates the patient, family members and caregivers on disease processes, self-care techniques and prevention strategies, coordinates of patient care and services with the physician and other health care team members.
- Physical Therapy/ Occupational Therapy / Speech Therapy provided by a licensed therapist or therapy assistant. Therapist will provide specific information about the services and treatment the patient will receive. Medically necessary therapy services must be coordinated with one agency while the patient is receiving medically necessary home health services.
- Home Health Aide Services delivered under the licensed supervision of a registered nurse. Typical duties include bathing, shampooing hair, changing bed linens and assisting with other activities of daily living.
