Provides primary medical health care services to families in need. Services provided cover preventative and acute medical illnesses and conditions. 

The Immunization Clinic provides immunization for most vaccine-preventable diseases in both adults and children.

Screening and treatment for cardiovascular diseases, diabetes, cholesterol, and depression.

Routine services such as pap smears, school physicals, prenatal visits, well baby exams, physicals and other routine healthcare procedures. Follow-up visits for patients with diabetes, elevated blood lead levels, hypertension, behavioral health concerns, and other on-going health problems are also routine activities at the clinic.

Health education is integrated into each patient's visit. Nutrition, disease prevention and management are taught to patients individually and in groups. Educational follow-up, care coordination, management and peer education, including care for sexually transmitted diseases, latent tuberculosis, high lead levels, prenatal care, and care for chronic diseases (asthma, diabetes, hypertension, high cholesterol, and depression).

Specialist referrals for services not available at the clinic are made for all patients. Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.

- Education on the dangers of radon gas and radon testing kits.

- Education and colon cancer screening kits to the community to encourage prevention and early detection of colon cancer.

- Tai Chi and Bingo-cize fall prevention classes.

- School nursing.

- Substance abuse prevention through the Healthy Communities Initiative, providing education and activities to reduce drug and alcohol use among youth.

- Dental screenings and education offered to preschool and grade school children. Sealants and fluoride treatments also available.

- Childhood and adult vaccines, provided at each Public Health Department location (Norfolk, Tekamah, and Wisner). Includes vaccines for those who are privately insured or receive Medicare or Medicaid, and those who are uninsured.

Provides mammograms and Pap tests for early cancer detection, lead poisoning screenings for young children, and home health aide services to support recently discharged hospital patients, elderly individuals, and people with disabilities. Works closely with the county Board of Health and serves as a funding source for community needs assessments. Also provides information about communicable diseases.

Also provides the HealthySteps program, for children ages 0–5.

Provides childhood, adolescent, and adult immunizations to those in need who live in Hardin or Grundy County. Flu vaccines may also be available. 

Offers home care, public health, immunizations, mental health administration, early childhood development, and environmental health services. Provides information on communicable diseases, routine general physical examinations to assess overall health and detect abnormalities, and birth control services to help individuals prevent conception

Provides prenatal care to ensure the health and well-being of pregnant individuals and their developing children through risk assessment, routine checkups, prenatal testing, and guidance on pregnancy-related concerns.

Provides well-baby care services, including health screenings and immunizations, to support the physical well-being and healthy development of infants and toddlers, with some programs extending to preschool-aged children.

Also offers comprehensive preventive, diagnostic, and treatment services for sexually transmitted infections, including contact tracing to help prevent the spread of infections.
