Provides health care services to those experiencing acute or chronic pain and/or illness, or those seeking medical services that are not due to an emergency. Services include annual screenings or check ups, chronic disease management, immunizations, pregnancy tests, pre and postnatal care, school and sports physicals, sexually transmitted infection tests, and well-child exams. 

Provides public health services to Winneshiek County residents and will make referrals as needed. Services include health education, CPR, Stop the Bleed training, childhood/adolescent/adult immunizations, testing for Sexually Transmitted Infections and rapid HIV testing, blood pressure checks, and much more.

Immunization administration and immunization education are provided by appointment.

Refugee Assessment and Immunization Clinic provides the required medical assessment and vaccines to newly arrived refugees of all ages within the first 90 days of arrival in the United States. Referrals to private medical providers for ongoing services are made. This program works in collaboration with local refugee resettlement agencies.

Health center for care of acute and chronic illness. Services can include women's health care services, diabetes management, immunizations, flu, and pneumonia shots, infant checkups, pregnancy testing, physicals for school, sports, and work, and sexually transmitted disease tests and treatment.

Childhood immunizations, some adult immunizations, flu, and Covid immunizations. Breastfeeding assistance and education, parenting classes, TB treatment, and assisting families in finding medical and dental care. Case management is provided to reduce lead and other household environmental exposures, such as Radon.

Public Health Emergency Preparedness planning with communities, businesses and healthcare partners.

West Nile Virus surveillance to monitor the presence of the virus in the communities and educate about protection from the virus. Resource and Referral to connect people to available services. Minority Health Initiative to assist local minority populations with health education and access to healthcare services. Infectious Disease Surveillance and Investigation to prevent and protect the public from infectious diseases. Radon education and source for low-cost home test kits. VetSET to connect veterans and their families to available resources. Health Literacy promotion, education, and resources to assist people to better understand their healthcare information. Community Response in collaboration with the Wayne County Family Coalition to assist families living in Wayne County to available resources.

Offers vaccines and information for individuals in need to answer questions, or misinformation about certain vaccines. Services include flu vaccines, COVID-19 vaccines, and other vaccines may be offered under this program. 

Offers sexually transmitted disease (STD) testing and treatment, HIV counseling and testing, communicable disease reporting and investigating, immunizations (including flu shots) for children and adults, Tuberculosis (TB) screening and treatment, and international travel information.

Offers primary care services to those in need. Services include birth control counseling, childhood immunizations, confidential HIV testing, drug and alcohol testing, general physical examinations, gynecology services, pregnancy testing, seasonal flu vaccines, sexually transmitted disease screening, and well baby care.

Provides preventative reproductive health care and education, including contraceptive education, sexually transmitted infection prevention, and pregnancy-related information.

- Offers breast examinations, Pap tests, and instruction on breast self-examination.

- Provides general immunizations to prevent diseases and reduce health risks for individuals and the community.

- Shares information on environmental hazards and safety measures to promote overall health.

- Offers blood pressure screenings to identify risk of hypertension-related conditions.

- Provides referrals to programs like WIC, to support nutrition, breastfeeding, and overall health.

Provides child vaccines. All scheduled childhood vaccines are available. Immunization for infants and children (6 weeks to 18 years of age) for uninsured, underinsured, and Medicaid/All Kid's recipients. 8 clinics per month, 6 total sites located throughout McHenry County

Adult Vaccines, all recommended vaccines for adults (19 years of age and older) for both uninsured and underinsured. 2 walk-in clinics per month. Travel vaccines are not offered.

Clinics are offered for infant, child, adolescent and adult immunizations.

Other adult and child immunizations provided include tetanus, diphtheria, pertussis, polio, HIB, hepatitis A & B, measles, mumps, rubella, varicella, infant pneumococcal disease, and rotavirus infant diarrheal disease. Meningococcal Vaccine for persons aged 11-18. HPV (Human Papillomavirus) Vaccine for Children 9 –18 years of age. School-based Immunization clinics Influenza and pneumonia clinics (in-home service if necessary) offered, visit website for days and times for clinics.

Offers adult and child immunizations. Influenza shots are offered during the flu season (October through March).
