Care coordination team works together with families to develop a plan of care that addresses a child’s medical, social, behavioral, educational and financial needs. Care coordination is free for all children who have eligible medical conditions, regardless of their family’s income level.

Care coordination can include finding specialized medical care, helping families understand their child’s diagnosis and medical treatment plan, explaining insurance benefits and attending school meetings.

Helps children who are medically fragile remain in their family home rather than a hospital or skilled nursing facility.

Assists with:

- Finding specialized medical care

- Making sure families understand their child’s diagnosis and medical treatment plan

- Helping families understand their insurance benefits and maximize their coverage

- Attending IEP /504 Plan school meeting

- Connecting families with financial support, grants and other community resources

Provides caregivers of adults aged 60 and older with temporary relief from the stresses and responsibilities of constant care for functionally impaired older adults. Services include the provision of a caretaker for the impaired older adult in their own home or in a community setting or facility, offering a brief period of rest or respite for the caregiver.

Provides foster care and foster parents support with an emphasis on children with disabilities (though there are also children without disabilities in the program). Services include:

· Foster home licensure.

· Children's placements.

· Case management.

· Social work services.

· Foster home training.

· Respite Care.

· Adoptions.

· Family Support.

Offers services that include errands, shopping, personal assistance, house-sitting, in-home assistance, referral services, concierge services, personal care, senior case management, nursing and medication setup, companionship, respite care—including temporary or intermittent care for dependent adults in a community setting or facility—and much more.

Provides non-medical in-home assistance to the elderly or disabled. Specializing in Alzheimer's and Dementia care as well as Parkinson's care. Offers help with personal care such as bathing, toileting, dressing, ambulating or walking, and eating. Also offered is companion services such as preparing meals, light housekeeping, doing laundry, providing transportation, and just being a companion.

Provides support services to individuals caring for a veteran with serious illness or disabilities. For qualifying individuals, access to training, counseling, or respite care may be available.

Provides help when a family is in crisis, kids can become vulnerable to neglect and abuse. Through the Safe Families ministry, volunteer Host Families offer loving care to parents, watching over their children while the parents regain stability.

Provides an opportunity for a caregiver to receive appropriate, temporary, substitute care and supervision of a functionally impaired person. Aim is to enable the caregiver to maintain his or her support/assistance to the older person. Respite services can be used for a variety of reasons such as a day out with friends, to attend church, vacation, and attend support group meetings.

Provides help with daily activities, including companionship, personal care and hygiene, medication reminders, staying active, grocery shopping, light housekeeping, meal prep, transportation and errands, hobbies, meaningful activities, etc.
