Offers a Disability Access Point, or “DAP”, which help people with disabilities, their families, and their caregivers find the services and support they need—both short-term and long-term. DAPs help by giving information, making plans, making referrals and checking in to make sure people are getting the right support to live healthy, independent lives.

Can help with the following services:

- Information and Assistance:  Based on presented and identified needs, provides information about programs and services, and helps to connect individuals to service providers in their communities.

- Options Counseling:  Provides guidance to individuals so that they may make informed choices about supports and services, typically over a period of 90 days or less. This includes benefits screening and application assistance for programs and services and follow-ups to make sure supports and decisions are assisting the individual.

Short-term supports (time-limited and must meet additional criteria) may include adaptive and assistive equipment, home and vehicle modifications, peer and parent support services, rent assistance, transportation assistance, and other basic needs (not covered by insurance).

Long-term services and supports offers coordination services including adult day programs, consumer-directed attendant care (CDAC) (non-skilled and skilled), day habilitation, employment like job skills training, prevocational services, and supported employment, intensive residential services (IRSH), personal response systems, respite services like in-home, in-facility, and day camps, supported community living (SCL) and home-based habilitation (hourly), and residential living settings.

• Case management and information on community resources
• Psychiatric services

Offers several levels of case management care that are intended to support and enhance emergency services currently available in the Region V service area. Provides assistance to help consumers achieve and maintain a higher quality of life.

Program is a collaboration between Houses of Hope, Lutheran Family Services, Blue Valley Behavioral Health, The Bridge Behavioral Health, and Region V Services.

Assists individuals with mental illness, intellectual disabilities, and developmental disabilities in accessing needed services and supports. Coordinators provide information, referral, assess individuals for needs, develop a treatment plan, and coordinate funding as needed.

Intensive case management for those in need of mental health and/or substance abuse services.

Comprehensive treatment program for individuals who have experienced a first episode of psychosis. A team of providers collaborates weekly on supporting the clients in their efforts to function at home, on the job, at school, and in the social world. Services include outpatient mental health services (a psychiatric provider and an individual therapist), a family education specialist, supportive employment and education caseworker.

Provides services to individuals with mental health issues. Services include case management, counseling, prescription administration and representative payee services.

- Professional Partner Program: Provides case management to families with children experiencing behavioral or emotional concerns, utilizing a family centered philosophy.

- Rental Assistance: Offers an 18 month rental assistance program with a declining subsidy for transition age youth (ages 18-26) who are enrolling or are enrolled in the Professional Partners Program.

- System Coordination: Manages a local network of behavioral health providers focusing on the areas of network management, coordination of emergency, prevention, consumer and youth services.

Arm in Arm, Aiding Recovering Moms is a program for pregnant and/or parenting women who also have problems with substance abuse. Substance abuse recovery planning coordinates schools, employers, family, and recovering person to put a plan in place that includes relapse prevention. Youth care coordination program offers one-to-one support and case management services for children and youth with a severe emotional disturbance (SED) diagnosis. Community support assists persons with severe and persistent mental illness through one-to-one services, helping clients live in a residence of their choice. Provides assessment, planning, linking and monitoring services to meet the client's needs. Prevention services include a wide-range of speakers, informational videos, books, and brochures.

Emergency support program provides support to individuals who are experiencing a mental health or substance use emergency.

Medication Assisted Treatment including Medication Management with providers specially trained and licensed to provide MAT.
