HOMELESS STREET OUTREACH | THRESHOLD COC
Assists the needs of people living on the streets (outside, abandoned buildings, in cars, etc.) in the Omaha Metro area who are interested in connecting with housing and/or other services.
Services provided are:
- Screening and referral for housing programs
- Information and referrals for mainstream benefits (SNAP, Medicaid, GA, etc.)
- Emergency supplies (food, water, blankets, hygiene items)
- Connection to emergency shelter, if desired
- Case Management
- Basic health assessments and connections to long-term mental and primary healthcare
