Contact office for intake procedure, no referral is needed to consult with Hospice staff to determine whether a patient fits hospice criteria but a physician referral is needed to begin hospice services

Mary Greeley Hospice Care has Home Based Hospice and Israel Family Hospice House. Services also include bereavement counseling, bereavement support groups and respite care.
