Eligibility
Only able to accept new patients age 0-18 who are Champaign and Urbana residents, Medicaid recipients (all MCO’s), are actively enrolled in WIC, foster children who live in Champaign County, patients seen at school by CUPHD dental staff through the school dental sealant program within the last 2 school years

Dental Clinic: Completed Patient Registration Packet (available in office or on website), photo ID, insurance card(s)
School Dental Sealant Program: Consent form (available in office or on website

Dental Clinic:
Provides a full range of dental services for local youth, including:

-- Space maintainers
School Dental Sealant Program:
Conducts visits to all public schools in the area served to provide school dental exams, cleanings, fluoride treatments, and sealants.
