Provides shelter, assistance, and 24-hour case management to those experiencing a need for transition to another home or residential setting or who need a break from their current home or residential setting.

- Provided on a short-term basis, to provide support through the transition or break. Provides links to needed behavioral health services and assists in the transition back to the community.

Services include:

- Case Manager, providing individual and group education services.

- Certified Peer Support Specialists, offering individual and group recovery services.

- Residential Technicians, providing individual and group services.
