FAMILY UNIFICATION PROGRAM at DES MOINES MUNICIPAL HOUSING AGENCY
Vouchers will be used to promote family unification of families for whom the lack of adequate housing is a primary factor in the separation, or the threat of imminent separation, of children from their families. Family must be involved with the Child Protective Services Unit or an individual currently 18-21 years old that left foster care at age 16 or older. FUP Vouchers provide rental assistance to eligible applicants who wish to rent from a private property owner. If you feel you meet one of the above criteria, applications may be picked up and returned to the Iowa Department of Human Services main office at 2309 Euclid Avenue, Des Moines, Iowa 50310. The DMMHA does not have the Family Unification Program applications at its Administrative Offices.

Eligibility
Family is involved with the Child Protective Services Unit OR an individual 18-21 years old that left foster care at age 16 or older. Income guideline: 1 person - $28,800, 2 - $32,900, 3 - $37,000, 4 - $41,100, 5 - $40,450, etc. Family composition, arrest history, and past participation in Federally subsidized programs are also taken into account
