Child and Adolescent Health Program | Upper Des Moines Opportunity, Inc.

Provides health services and support to help children and youth stay healthy and access needed care. Services include health screenings to assess overall health and development, dental screenings and education, immunizations, developmental and behavioral screenings, Medicaid and Hawki application assistance for children who may qualify, health information for families whose children are newly enrolled in Medicaid, care coordination to help families access regular medical and dental care, connections to community resources based on family needs, and Child Care Nurse Consultant services to support the health and well-being of children in child care settings.
