1st Five is a public-private partnership bridging primary care and public health services in Iowa. Enhancing well child exams through developmental surveillance and screening. The 1st Five model supports health providers in the earlier detection of social-emotional and developmental delays and family risk-related factors in children birth to 5 years old. Referrals are made to the 1st Five program by local medical providers. 1st Five Staff contacts the family to coordinates referrals, interventions and provides follow-up for both medical provider and family
