Visit website, or visit in person to complete a Financial Affidavit form, then the Collections Administrator will request a driving report from the Iowa Department of Transportation and review the information to determine eligibility, applicants will be contacted with the next steps after the review

Provides a driver's license reinstatement payment plan for qualifying Warren County residents with a suspended driver's license. The County Attorney Payment Program (CAPP) allows eligible individuals to set up a payment plan for delinquent fines, which may help them regain their driving privileges. Applicants must complete and return a financial affidavit to the Warren County Attorney's Office. Eligibility is determined after the financial affidavit is reviewed. Individuals are not eligible if any of the following criteria apply:

- License suspended for non-payment of Child Support

- License suspended for out-of-state tickets

- Failed to complete a substance abuse evaluation (if required by the DOT)

- Failed to obtain an ignition interlock device (if required by the DOT)

- Failed to complete the Drinking and Driving Class (if required by the DOT)
