Offers to assist people with severe disabilities so they can remain in their homes and be as independent as possible. The program tries to prevent unnecessary nursing home placement by offering in-home care.

Services include personal assistant services (personal care, household tasks, and certain health procedures), homemaker services, maintenance home health, electronic home response, home delivered meals, adult day care, assistive equipment, home modifications for increased safety, and respite services.

Services are also available for persons with HIV/AIDS and/or traumatic brain injuries.
