Eligibility
Open to those who have trouble hearing on the phone; Must have either a landline and/or cell phone; Doctor or audiologist must complete the back side of the application certifying that the applicant is hard-of-hearing
Required documents
Driver's license, State ID, or piece of mail showing the same address as the application
- Copy of the most recent landline phone bill and/or cell phone bill.

Description
Selection center for the ITAC Amplified Phone Program. Those that have trouble hearing on the phone may be eligible for a free amplified phone and/or cellular amplifier. Individuals can receive an application packet, test, and select the equipment they would like to order at LIFE CIL.
