Eligibility

Open to individuals who are uninsured or underinsured, or open to clients who cannot afford SamaraCare’s standard fees due to an existing financial hardship or change in their financial circumstances

Required documents

Proof of income for all individuals living in the household, provide proof for financial hardship (e.g. divorce or recent separation: copy of divorce decree or separation agreement, insurance-related (i.e., high deductible, no mental health benefits, mental health benefits exhausted): copy of Form 1095-A (health insurance marketplace statement or, Form 1095-B (health coverage) or Form 1095-C employee-provided health insurance offer and coverage), death of primary wage earner: copy of death certificate or obituary, bankruptcy: copy of bankruptcy settlement discharge papers, high medical debt: proof of all outstanding debts or medical bills (copies of bills, statements, late notices, etc.)

Provides counseling services to those in need who are considered low income. Services are based on a sliding fee scale to make services more accessible for those without insurance or underinsured. Also provides fee discounts on psychiatric services to clients who self-pay, due to insurance limitations or being out of network.
