Eligibility

Must be either pregnant, breastfeeding, or less than six months postpartum; or caregivers with children age infant to 5 years old, must meet income guidelines, 185% of the poverty level

Required documents

Your child, proof of residential address (Medicaid can no longer be accepted as proof of residency), Medicaid or 2 recent pay check stubs for each person in the household, immunization records (if child receives immunizations somewhere other than the McLean County Health Department)

Application process

Visit in person only, however, initial visits for pregnant women require an appointment, those in need of interpretation services need to make an appointment
