ID for responsible person and participant (i.e. drivers license, birth certificate, etc.), proof of residency (i.e. piece of mail), proof of income (i.e. check stub or Medicaid card), and immunization record for child

Eligibility

Must be pregnant, breastfeeding, or recently had a baby, infants and children under 5 years of age; income must be within guidelines; families who receive ADC, SNAP, Kids Connection, or Medicaid are automatically eligible; must have a nutritional risk
