Proof of identity/age: valid driver's license, passport, or birth certificate; If a birth certificate is used it must be a certified copy; If proof is not furnished, the license cannot be issued; Contact our office for questions concerning identification papers

Must be at least 19 years old; minors who are 17 or 18 years old may apply with parental or legal guardian consent (unless an emancipated 17 or 18 year old minor; emancipated minors are required to provide court order declaring they are emancipated)
