Applications are also available at county treasurer offices, driver license exam stations or Department of Human Rights, Persons With Disabilities Division. Must have a signed physician's or chiropractor's statement on the physician's or chiropractor's letterhead stationery verifying the person's disability and whether the disability is temporary or permanent.
