Accepts Medicaid, AllKids (CHIP), Title 21, and Bloomington or Normal Township vouchers, clients who pay for services out of pocket must meet financial guidelines before treatment can be approved, out of pocket fees are based on current Medicaid reimbursement rates
Required documents
Depending on the patient receiving services, please present Medicaid card, individuals on General Assistance should bring their Bloomington or Normal Township voucher with them
Eligibility
Open to children and youth up to age 20, meet income requirements, must have a medical card or be a recipient of general assistance
