Eligibility

Open to individuals living with HIV/AIDS; Individuals who meet income and insurance guidelines are also eligible for dental care, housing services, utility support, primary HIV medical care, nutrition services, vaccinations, transportation assistance, mental health services and other services; Contact CUPHD or HIV Care Connect for more information regarding eligibility
