FAMILY CASE MANAGEMENT | MCHENRY COUNTY DEPARTMENT OF HEALTH
Offers services to promote optimal health and provide support for pregnant women, infants, and children with high-risk medical conditions by encouraging early prenatal care and making referrals to community and social services.
Family Case Management assists with understanding the importance of prenatal care, finding a doctor for prenatal care, understanding proper nutrition for both mother and child, understanding the stages of child development, learning parenting skills, recognizing the signs and symptoms of preterm labor, understanding the importance of well-child visits and immunizations, and connecting to additional resources.
Public Health Nurses and Case Managers provide services in the office or at home, offering information on prenatal needs, infant care, parenting, and household safety tips to protect infants and children. Additionally, guidance is available on child passenger safety, including selecting an appropriate car seat based on a child's height and weight, correct car seat installation, and ensuring proper child restraint in vehicles.
