Application process

Referral form available online (Google Chrome or Microsoft Edge recommended for form completion); Referrals accepted from: - Children being treated for a critical illness - Parents or legal guardians - Medical professionals such as providers, nurses, social workers, or child-life specialists - Family members with detailed knowledge of the child's current medical condition If the referral form is completed by an unapproved referral source, the request is not considered an official referral but a request for additional information

Eligibility

Children must meet the following criteria at the time of referral: - Must be older than 2 1/2 and younger than 18 - Must live in Illinois - Diagnosed with a critical illness (i.e., a progressive, degenerative, of malignant condition that is placing the child's life in jeopardy); The child's treating physician makes the final determination of whether a child's medical condition qualifies him or her for a wish - Must have not received a wish from another wish-granting organization
