Must be age 18 and older, have a diagnosis of mental illness, willing and able to take part in a variety of mental health services, free of communicable diseases and medical needs that require nursing care, must have Medicaid or ability to qualify for Medicaid

Provides residential services to homes that house individuals who require mental health assistance. These residents require help to function in their daily lives and work to develop learning skills needed to achieve an optimal level of independence. Services include psychosocial rehabilitation (PSR), case management, individualized placement support (IPS), and psychiatry.
