C-U AT HOME PATHWAYS TO PROGRESS, MEN'S PROGRAM | C-U AT HOME

Works to end homelessness one person at a time. Offering seven homes across the community, where individuals enrolled in our 12- to 18-month Pathways to Progress Program receive support. This intensive program follows a four-phase system, addressing seven key areas of instability to help participants regain stability and independence.

Must be ages 18 and older and homeless or near homelessness, Must be willing to do an intake and provide information such as: demographics, substance abuse, mental health and criminal history, Must be willing to work with the case management team and develop a case plan
