When calling for a reservation, be prepared to give the following information, Name, address, 1- or Multi-Way Trip, Telephone Number, Alternative Number, Date of Appointment, Time of Appointment, Address of Destination including zip code, Emergency Contact (and phone number), Is the rider mobile or do they use a wheelchair? Will the rider need a chaperone/aide (non-paying) to travel with them?
