FAMILY SERVICES | CARPENTER'S PLACE

Provides case management on a long-term basis, up to one year or longer and as needed, to assist the family in overcoming the circumstances that brought them into an "at risk" position with homelessness, financial instability, less-than-living-wage employment, lack of education, substance abuse, mental health instability, physical health instability, legal issues, need for counseling, domestic violence issues, lack of child care, lack of transportation, and lack of basic household items, food, clothing and personal items to sustain appropriate health.

Application process

The family must be referred in writing, using a Carpenter's Place referral form, by a Winnebago, Boone or Ogle counties partnering agency, entity or community group; Families, friends, state or national agencies are not acceptable referral sources for this program

Illinois ID or DL for head of household, Social security cards for all family members, Birth certificates for all children in household

Eligibility

Restricted to families who have at least one adult over the age of 18 and at least one child under the age of 18 OR, have a child coming to the home in the next 180 days (ex. from birth, foster care, hospitalization, or from some other type of an institution) OR, have an adult child over the age of 18 who qualifies as special need who is unable to live alone due to medical, developmental or mental health issues (must be receiving disability)
