Eligibility

Open to mothers, must be either pregnant, breastfeeding, or less than six months postpartum, or caregivers with a child ages 0-5, must meet income guidelines, 185% of the poverty level

Required documents

Proof of income (paycheck stubs, income tax return, or proof of current eligibility for Medicaid, Food Stamps or FIP), proof of address (utility bill, rent receipt, DL., or voter registration card with address), proof of identity for each family member (DL, birth certificate, insurance card, school or work ID, crib card, or SS card), Immunization record for children 2 years and younger
