COMMUNITY CARE PROGRAM | CCSI CASE COORDINATION
Provides in-home assistance to seniors to help them remain independent in the home. Services include case management, home care aid (non-medical in-home care to assist with activities of daily living, such as bathing, grooming, dressing, housework, meal preparation, and transportation for essential errands), emergency home response system, automated medication dispenser, and money management.
Care Coordinators can also assist older adults and their caregivers in finding options when leaving the hospital or long-term care center.

Eligibility
Must be ages 60 and older, be either a U.S. citizen or eligible non-citizen within the specific categories, be a resident of Illinois, have non-exempt assets of $17,500 or less (home, car, or personal furnishings are classified as exempt assets), have an assessed need for long term care (to be at risk for nursing facility placement as measured by the Determination of Need (DON) assessment)
