ADULT DAY CENTER | HEARTLAND OF STORY COUNTY

The Adult Day Center supports individuals with dementia, Parkinson's disease, and chronic health conditions that impact personal safety and limit them from staying home alone during the day. ADC offers a safe, environment by providing a variety of activities, meals/snacks, and individualized health monitoring. ADC also allows caregivers the respite they need to tend to other needs.

Each participant has an individualized plan of care developed with the involvement of family members and caregivers. ADC is a great alternative for those seeking a high-quality, cost-effective alternative to nursing home care.
