Medicaid riders, if your ride is covered by Medicaid, your ride must be booked through your Medicaid transportation broker. Rides cannot be scheduled without Medicaid broker approval.

To schedule visit the website or call at least one day in advance to receive priority consideration. Same day ride requests are subject t o driver availability. Have the following information when you schedule a ride:

Name and contact information

Date of birth

Pick up address

Drop off address

Reason for ride, if it's for an appointment, time of appointment

If you need assistance boarding vehicle
