Completed online referral by the Doctor or Social Worker and completed an application by the parent or guardian.

Eligibility

Have child under the age of 18 who has been diagnosed with cancer, and who is in an active treatment regimen; Must be traveling at least 60 miles, one way, to access cancer treatment; Family can demonstrate a financial need, family and child must live in the Contiguous United States and the child must be treated in the Contiguous United States
