Children, birth up to age 21, with or at risk of a chronic health condition or disability which includes psychosocial, physical, health-related, educational, and behavioral needs; the specific health concern may be simple or complex; serves Black Hawk, Bremer, Buchanan, Clayton, Delaware and Fayette counties; other counties on a need basis
