Open to children ages 3-5, income eligible and/or if the child is foster child, homeless, family receiving public assistance or disabled

Required documents

Child’s birth verification (Birth Certificate, Title 19/Medical Card, or Passport) Proof of income for the last calendar year or 12 previous months (copy of pay stubs, taxes, W-2, letter from employer, FIP, Public Assistance Benefit Letter or other form of verification), Birthday for all family members in the household
