BRAIN INJURY PROGRAM | CENTRAL ILLINOIS SERVICE ACCESS

Provides case management to assist individuals living with a brain injury obtain services. Goal of the program is to maintain individuals in their homes and prevent institutionalization while increasing independence and improving the quality of life. Program can provide personal care workers, nursing care, therapies, home modification, emergency home response, and a variety of other services.
