THE CONTINUUM OF CARE - SPECIAL NEEDS ASSISTANCE PROGRAM (SNAP) | DUBUQUE CITY OFFICES

A permanent supportive housing program that assists those who are chronically homeless and also have either a substance abuse issue, mental health diagnosis and/or has been diagnosed with having HIV/AIDS. SNAP provides rental assistance for homeless persons with disabilities in connection with supportive services funded from sources throughout the community through other agencies and programs.
