Eligibility

Must meet income guidelines and be a McHenry County resident for at least 90 days

Current Illinois State-Issued Driver's License or Photo ID, proof of being a McHenry resident for at least 90 days, social security cards for all household members (or written verification from the Social Security Administration), bank statements for the last 3 months (checking and savings), all documentation of gross income for 90 days

Application process

Call to set up an appointment, approved applicants will be given a voucher to use the dentist of their choice who practices in within the state of Illinois, assistance is limited to no more than $1000 per person

Description

Offers financial assistance to those who are income eligible and who have no insurance or other methods in which to pay for needed dental work. Applicants must be experiencing current dental pain and/or have difficulty eating/chewing, or the applicant must have missing/broken and/or decayed front teeth with such an appearance that the condition hinders the applicant's ability to obtain employment.

**The dentist being used must be willing to accept a voucher and to send the bill directly to MCHA.
