Services include care for illness and injury, medication assistance, flu immunizations, TB screening, smoking cessation, women's health, mammograms, wellness checks, lab services, chiropractic, physical therapy, health planning, vision, dental services, pediatric care addressing the physical, emotional, and social health of children and adolescents, and podiatry services for the prevention, diagnosis, and treatment of foot-related conditions.

Eligibility

Must be a resident of/work in area served, at or below 250% Federal Poverty Level, does not receive Medicaid, Medicare, or other subsidized programs and does not have 3rd party insurance (this may be waived if the patient meets income guidelines)
