DSCC guides families through their child's journey with a medical condition. Works with doctors, schools and community groups to create a seamless support system.

DSCC's care coordination team works together with families to develop a plan of care that addresses a child's medical, social, behavioral, educational and financial needs. Care coordination is free for all children who have eligible medical conditions, regardless of their family's income level.

Care coordination can include finding specialized medical care, helping families understand their child's diagnosis and medical treatment plan, explaining insurance benefits and attending school meetings.

Helps children who are medically fragile remain in their family home rather than a hospital or skilled nursing facility.

Referrals accepted from anyone with parental permission or from parents themselves

Fee

Financial assistance is based on parental or client income and available private insurance must be utilized prior to expenditure of agency funds; care coordination and benefits management is provided to medically eligible children regardless of financial eligibility; diagnostic evaluations are provided regardless of income

Application is available online and specifies what documentation is needed; if financial assistance is needed/requested, proof of income is required

Open to youth ages 0-21 with a medically eligible condition as well as children and adults in need of skilled in-home nursing
