GROUP HOME SERVICES at RESIDENTIAL CENTER
Services for people with an intellectual disability and who may also have a physical disability. The program consists of Residential Care Facility-Intellectual Disability (RCF-ID) licensed group homes housing eight individuals with intellectual or developmental disabilities. Staff provide supervision, instruction, and assistance in community living. Residents receive 24-hour supervision and assistance and must participate in daily work or day habilitation services.

Referral through the Department of Human Services (DHS), schools, Scott County/Region or self; review of background and several visits prior to move
Fee
Room and board payments according to facility, third-party funding or sliding fee scale for support services; funding available through Home and Community Based Intellectual Disability or Brain Injury (HCBS ID or BI) Waivers, or Scott County/Region
Required documents
Social history, physical exam, Physician's Level of Care statement, and evidence of intellectual or development disability as well as an authorization for approved funding
