Transition and Eldercare Services assist seniors and families as they move between care settings, such as from hospital to home, or explore long-term care options. The program focuses on safety, continuity of care, and connecting individuals with the supports they need to age comfortably and confidently.

SHIP (Senior Health Insurance Program) offers free, unbiased assistance to help seniors understand their Medicare options. Trained counselors guide individuals through coverage choices, enrollment, and benefits, ensuring older adults can make informed decisions about their health insurance.
