Provides help persons with disabilities to find and acquire full or part time work in their communities. Offers a day program designed to give persons with disabilities the opportunity to grow skill sets and explore life, no matter how severe their disability.

Services include:

- Day Programs.

- Work skills evaluation.

- In-house/on-site community job placement.

- Vocational case management.

- Young adult employment services.

- Transportation services.

- Life skills mentoring.

Free round-trip handicap-accessible transportation for Council Bluffs patients from residence or place of employment. Also offers delivery of All Care prescriptions.

Offers demand-response, door-to-door transit in Fremont. All vans are wheelchair accessible.

Provides accessible transportation to the general public that travel within or to Piatt County.

PiatTran’s bus fleet is fully accessible with lifts and securements for all types of mobility devices.

Residential services include Traditional Group Homes, Community Support Program, and Supported Community Living Services.

Traditional Group Home offers training in self-help skills, household management and leisure-time skills. These programs integrate people into the community in the least restrictive environment. All individuals are encouraged to have an outside activity. Group home sites are in Red Oak, Malvern, and Shenandoah.

Community Support Program teaches independent living skills to persons living in their own apartment or home. Individuals receive additional support services as needed such as transportation, personal shopping, meal planning, socialization, self-help skills, household management skills, and money management.

Supported Community Living Program provides up to 24 hours of support to assist individuals with their daily needs.

Respite Care Program provides short-term relief in the absence of the family or caregiver, such as a vacation or a night out.

Paratransit transportation for Siouxland area including, but not limited to, individuals with disabilities:

-- Individuals must complete an application for assistance and 21 days must be allowed to process the application.
-- Door-to-door service is available and costs $3.60 each way.
-- If the individual can get to the curb, then curb-to-curb service costs $3.60 each way. Siouxland Regional Transit System (SRTS) is a demand-responsive, ADA accessible, public transit system which provides public transportation services to the general public of Cherokee, Ida, Monona, Plymouth and Woodbury counties in Iowa and Southern Union County in South Dakota. Buses are equipped with a mobility device lift.

-- Curb-to-curb is defined as the individual waiting at the curb when the vehicle arrives and the individual is dropped off at the curb.
-- Door-to-Door service is defined as the individual waiting outside the door, and will be delivered to the door at his/her destination.
-- Each rider is allowed one (1) personal care attendant, and this person will ride free of charge. The attendant must be assisting the client or will be charged for the trip. Please schedule the additional person when making the reservation.

• Transportation assistance for low-income disabled Polk County residents and individuals 60 & over whom have no other means.
• A home delivered lunchtime meals for those unable to prepare a meal for themselves or transportation to a senior meal site.
• Financial management and budget counseling for low-income individuals

Provides comprehensive services for adults with intellectual or developmental disabilities. Services are designed to promote growth and support for individuals as they meet their individualized goals at home, work and within the community. Services include vocational, day habilitation, residential, case management, transportation, and therapeutic recreation services. 

Offers a variety of home based services dependent on individual personalized plans and needs. Available home based services include:

- Staff assistance with teaching various daily living skills.

- Transportation to medical appointments and other community resources.

- Accessing recreational opportunities in the community.

Emergency, critical care, and non-emergency ambulance transportation. Select ambulances are equipped with bariatric rigs that have ramps and winch to transport large patients.

Offers clinical rides for students at community colleges.

Provides transportation for people whose condition or disability prevents them from using the Metropolitan Transit Authority fixed-route buses. Services also include transportation for seniors ages 60 and older.
