SSVF is a Veterans Affairs funded program which is delivered through Primary Health Care. This program is focused on helping homeless and unstably-housed Veterans and their families living in Warren, Polk, Dallas, Jasper, Story, and Marshall Counties. Services provided include:

CASE MANAGEMENT services that will assist participants in obtaining VA and other public benefits, including: health care referrals, daily living referrals, personal financial planning, fiduciary and payee referrals, legal services, housing counseling services, employment and training referrals.

TEMPORARY FINANCIAL ASSISTANCE (TFA) which may include rent assistance, moving expenses, security and utility deposits, transportation, and child care. All financial services based on eligibility and available funding.

Aid and Attendance offers veterans and surviving spouses help with daily tasks in the home or assisted living. Veterans must have served at least 90 days during a period of war. If approved for the VA Aid and Attendance benefit, clients may choose any agency to provide the services, they are not obligated to choose this agency.

Service covered include help with daily activities such as dressing, bathing, meal preparation, and housekeeping.

Consumer/Finance - debt collection, repossession, garnishment, contracts/warranties, predatory lending, and public utilities (including utility disconnection or shut off notices).

Education - expulsion/suspension, special education, learning disabilities, school fees, access, vocational education, and student financial aid.

Employment - employment discrimination, wage claims, earned income tax credit, and taxes.

Family - adoption, custody, visitation, guardianship, domestic abuse, human trafficking and exploitation, family violence (child abuse, elder abuse, battered women/men), including assistance with restraining orders, and child support.
