Certified nurse practitioners provide a wide variety of health care services to diagnose and treat men, women and adolescents, including: adult physical exams, behavioral health consultations, pregnancy testing and family planning, well woman exams, STD testing, HIV testing, TB screening and treatment and smoking cessation. The team also provides treatment for chronic health conditions like diabetes, cardiovascular disease and asthma.

Provides skilled nursing visits, homemaker visits, blood pressure, flu, and immunizations clinics, and physical, occupational, and speech therapy. Also offers information on communicable diseases and public health responses.

Wellness program providing resources to manage the symptoms of arthritis focusing on exercises to help with cardiovascular endurance, strength, flexibility, balance and overall mobility.

Health care for all ages, including obstetrical, pediatrics, geriatrics, general medical care, women's health, reproductive health, prenatal, obstetrics and gynecology, and radiology/ultrasound.

Blood testing for lead.

HIV testing and counseling.

Tuberculosis prevention and case management.

Immunizations for children and adults, including flu vaccines. COVID vaccines available in office or at home.

Screening and treatment for cardiovascular diseases, diabetes, cholesterol, and depression.

Routine services such as pap smears, school physicals, prenatal visits, well baby exams, physicals and other routine healthcare procedures. Follow-up visits for patients with diabetes, elevated blood lead levels, hypertension, behavioral health concerns, and other on-going health problems are also routine activities at the clinic.

Health education is integrated into each patient's visit. Nutrition, disease prevention and management are taught to patients individually and in groups. Educational follow-up, care coordination, management and peer education, including care for sexually transmitted diseases, latent tuberculosis, high lead levels, prenatal care, and care for chronic diseases (asthma, diabetes, hypertension, high cholesterol, and depression).

Specialist referrals for services not available at the clinic are made for all patients. Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.

Activities related to the prevention and mitigation of the effects of chronic disease. Services include health clinic, health education, group exercise, physical fitness, etc.

Winneshiek County Public Health Nursing Services offers demonstrations and instruction to community groups in areas of CPR, first aid classes, farm safety, sun protection, infection prevention, and heart health.

Durable medical equipment rent/loan at Grand Island and Scottsbluff locations. Donations of equipment accepted.

Peer mentoring, where people with disabilities support each other in their daily lives.

Housing counseling assists people with disabilities in locating, acquiring, and maintaining safe, affordable, accessible housing.

Offers senior's an In-home health/mental health assessment, in-home mental health treatment, case management, and referrals to appropriate community resources. The Outreach Team consists of clinical social workers and a consulting psychiatrist to provide appropriate support services.

Has a directory of Lions Clubs and would refer based on where inquirer lives for possible assistance with glasses and hearing aid needs. Accepts donations of used eyeglasses for third world projects. Also accepts used hearing aids. Eye screenings for children 6 months to 48 months.

Focuses on the prevention, diagnosis and treatment of adult diseases including AIDS/HIV. Diagnosis and treatment can either take place in the office or a hospital setting depending on the severity of treatment needed. Can also receive AIDS/HIV preventative and treatment medications.

Provides no cost breast cancer and cervical cancer screenings. Mammograms and pap tests also available.

Offers partial foot health assessments from a skilled nurse. Services include foot assessments, blood pressure checks, pulse checks, lung assessments, general foot care and diabetic foot care, warm foot soak, trimming of nails/calluses using clippers or a dremel, lotion application/gentle massage, and education on proper foot care.

Affordable, comprehensive primary health care especially for those with limited resources. Includes general physical exams and immunizations. Services are provided regardless of ability to pay through a sliding fee discount program. Fees are calculated based on household size and documented annual income. All patients are eligible to apply for the sliding fee discount, even those with insurance.

Dental Clinic offers preventative dental services including: X-rays, exams, cleanings, fillings and extractions.

Pharmacy services offered to patients. Those eligible for the sliding fee program receive their medications at 30 to 50% below the retail market. Kohll's, Walgreens, Genoa, and Walmart fill all medications.

Mobile Medical Clinic services, in which a van visits different locations to see patients.

HIV/STD testing provides confidential HIV/AIDS testing and counseling.

Transportation program provides a limited number of cab vouchers.

Mental health services are provided in-house through a partnership with Lutheran Family Services of Nebraska, as well as being referred out to collaborating behavioral health agencies.

Prenatal care for women.

Radiology has certified x-ray technicians to provide limited x-ray services.

Medication assistance programs help uninsured and underinsured patients apply to various pharmaceutical companies. Staff complete applications for patients to obtain their medication at an affordable cost.

Translation services offered to non-English speaking patients. Languages include Spanish, Vietnamese, Arabic, Kurdish, Burmese, Karen, and Thai.

Offers people aged 60 and older a chance to sit down with a registered nurse and discuss their health care concerns in more detail. Each appointment is conducted by a registered nurse that has been trained in Health Assessment and can include individual health education on topics such as health history, breast self-exams, proper exercise and rest, cancer detection education, medication and more.
